  3. Full-Mouth Rehabilitation, Planned From the Final Teeth Backwards

Most implant treatment is planned from the bone up. Dr. Issa Abou Ayyash plans full-mouth cases the other way around — from the final teeth backwards. The finished bridge is designed first, and every implant position is calculated from that design before any drilling begins. This case shows how that prosthetically driven approach works, step by step.

The Starting Point

This patient had a severely compromised dentition — failing, worn, and missing teeth affecting chewing, comfort, and confidence. Keeping the remaining teeth was no longer predictable. The plan: remove the failing teeth and rebuild both arches on dental implants, designed around the final result from day one.

Step 1 — Design the Final Teeth First

Before surgery was even scheduled, the final teeth were designed digitally in 3D. Their exact position, shape, and bite were worked out in advance — because the teeth determine where the implants must go, not the other way around.

Step 2 — Plan Every Implant on 3D Imaging

Using the patient’s 3D scan and implant-planning software, Dr. Abou Ayyash positioned each implant virtually — checking angle, depth, and distance from nerves and sinuses, and verifying every position against the pre-designed teeth. When the plan was final, nothing about surgery day was left to guesswork.

Step 3 — Guided Surgery, Exactly as Planned

A custom surgical guide, made from the digital plan, directed the implants into their planned positions during surgery. This is where prosthetically driven planning pays off: precise placement, less guesswork, and a smoother recovery.

The Result So Far

The post-treatment panoramic X-ray shows the completed implant foundation: four implants supporting the upper bridge and six implants in the lower arch — each one where the plan put it, ready to carry the final teeth they were designed for.
